Change of Major Form

A Change of Major (COM) form must be submitted to the Undergraduate Registrar's office for each student adding or dropping Commonwealth College Honors, Departmental Honors or Interdisciplinary Honors. Select an option below to see an example of the correct way to fill out the form. In addition, there are special routing instructions below for the COM for Departmental Honors.

Departmental Honors Track: Special Routing Instructions

Please follow these steps when processing the Departmental Honors Change of Major forms to ensure the student's interaction with their Departmental Honors Coordinator:

DH Track Handout:

1. give the student a copy of the appropriate HN-CCDept track from our website
2. put a second copy of the DH track in the student's file
3. jot a note on the Action Sheet that the DH Track was given to the student

Change of Major form:

1. put an “X” on the form where the Departmental Honors Coordinator (DHC) must sign
2. look up the DHC's name on our website and write "referred to DHC [insert DHC's name]” along the left side of the Change of Major form
3. inform the student that s/he must a) obtain the DHC’s signature and b) then deliver the fully signed form to the Undergraduate Registrar's office so the honors designation may be added to her/his academic record
4. retain the the bottom (goldenrod) copy of the Change of Major form in the student's file and jot a note on the Action Sheet saying "COM routing via student to DHC and Whitmore"
