Materials for Program Partners

These materials are for IMPACT! program partners.

Student Service Contract
Students will complete the first half of the service contract and will then either email the form or deliver a hard copy of the form for the site supervisor to review and complete. The form should then be returned to the instructor to review and sign.

Student Evaluation
This form is to be completed at the end of the semester. Site supervisor feedback is very valuable and the instructor takes this feedback into account when determining the student's participation grades for the IMPACT! course.
